  • I thought Hospice was just for Cancer Patients?
    Not at all, there are many diagnoses that make a person appropriate for hospice services. Please see a List of Diagnosis from our Home Page.


  • How do I know if my loved one or I are Hospice Appropriate?
    Two Doctors must agree that a patient is Hospice appropriate: 1) The patients Attending Physician and... 2) the Hospice Medical Director. So if your unsure call our office and we will send someone out to talk to you.


  • Who pays for Hospice?
    Medicare 100%, Medicaid 100% (state specific), Private Insurance (most plans), Tri-Care/Champus, and Private Pay.


  • What % will I have to pay?
    None . If you are Medicare Part A or Medicaid there is no out of pocket expense to you.


  • What services do you provide?
    Services provided under Jordan's Crossing Hospice include: (1) Regular visits from the Hospice Care Team (Which includes the Hospice Physician, Nurse Case Manager, Certified Nursing Aid, Social Worker, and Chaplain.) (2) Medicines pertaining to the Hospice diagnosis and comfort medications. (3) The Durable Medical Equipment needed (Hospital bed, wheelchair, shower chair etc.) (4) other services include Medical supplies (diapers, blue pads etc.) and some supplements (ensure, glucerna etc.)


  • Can I still see my Doctor?
    Yes. With the Code Modifiers provided to your Physician by Jordan's Crossing Hospice you will still be able to see your Doctor and they can still be paid.


  • How long can I be on Hospice Services?
    Unlike other services such as Home Health, there is not a time limit for Hospice Services (as long as the patient is deemed appropriate, the services continue).


  • What area do you provide services for?
    We are allowed to cover a 50 mile radius from our Home Office at 624 Travis St., Suite 700, Shreveport, Louisiana. WE DO NOT CROSS STATE LINES! Please call our office to see if you are in our coverage area.


  • Why should I choose your Hospice over the other hospices in your
    When choosing a Hospice Service, you should also ask this question: Does your Hospice Physician make routine visits to my home? The answer to that question at Jordan's Crossing Hospice is YES!


  • How do I get started on you services?
    Just call and we will send someone out to talk to you right away.
