Who can I call to help me find out?

CALL JORDAN'S CROSSING HOSPICE!!! We will come to your home and conduct an Information Visit with you and help you determine which direction to go.  IF we feel your Loved One may be Hospice Appropriate, we will secure a Release to allow us to pull Medical Records.  IF those Medical Records indicate your Loved One is appropriate for Hospice Services, we will assist you with admission-IF your Loved One is not Hospice Appropriate, we will do our best to guide you in the direction you need to go, by providing you with a List of local Home Health Agencies or other Services that best suit your needs! We will never just abandon you--we will help you find help.  This is our commitment to you-you can count on it!
