So, How Does This work? How Do We Know Who Is Appropriate?

Your Attending Physician (Primary Care Physician) may refer you to Hospice Services.  Your Hospital or Clinic may also.  However, if you need help at home with a patient who has a terminal illness; chances are Hospice can help!!  We will be glad to send a staff member out to speak with you to provide an information visit, someone who can answer your questions about Hospice!!  As a courtesy to you, we will gather Medical Records and confirm appropriateness before moving forward.  If it is determined that your loved one is appropriate, we will advise you and your Physician!! 

 

Remember, Routine Home Care Hospice Services consist of routine visits by:

  • CNA's, Nurses, Social Worker, Chaplain and our Medical Director!!
  • CNA's will assist with your Activities of Daily Living (ADL's) such as bathing, dressing, and personal care needs.
  • Nurses will monitor your clinical status and keep your Attending Physician (if you choose) and/or our Medical Director  advised of your current medical status. 
  • Our Social Workers will visit to help you with arrangements and programs that assist you with financial matters. 
  • Our Chaplain is there for you with spiritual support, counseling, grief and bereavement issues. 
  • Our Medical Director, Larry C. Daniels, M.D. and our Nurse Practitioner, Connie Gannon, visit routinely with all of our patients IN THEIR HOMES!!!!  Think about it!!! NO MORE struggle to get your loved one to the Doctor's office---just to be confronted with a LONG wait.  Our Medical Director will be glad to coordinate care with your Attending Physician, so, there are no gaps in your care.  Only the BEST of the BEST for Jordan's Crossing Hospice Patients!!!   

Jordan's Crossing Hospice offers the public in North Louisiana the OPTION of remaining in their own HOME environment AND having supportive medical professionals come to them!  The ever-present availability of medical professionals brings great comfort and relief to an exhausted caregiver who wants only the best for their loved one.  In the home environment, Jordan's Crossing will provide a range of on-going services to the patient and the family unit.  We stand ready to assist patients and their families with access to available services and liaison associates with the medical community.
