The Hospice Physician manages each Patient's care and coordinates with the Attending Physician and Hospice Care Team.

The Administrator, Becki Prudhome, RN is responsible for JCH's compliance with all Federal & State regulations, laws, and procedures; directs the day-to-day operations of Jordan's Crossing Hospice.

IN ADDITION, Becki Prudhome, RN serves as Clinical Director, the Lead for Clinical Team who oversees and supervises all components of Patient Care on a day-to-day basis.

Registered Nurse/Case Manager 

coordinates the Care Team's provision of services to the Patient. Each RN is responsible for a Team of patients and oversees every aspect of the patient's care.

The Certified Nursing Assistant (CNA) or

Home Health Aide (HHA)

assists the Patient with Personal Care Needs (bathing, dressing, and other daily activities).

The Chaplain

Licensed Pastor & Director of Chaplain Services

provides spiritual counseling to the Patient and the Family based on the initial and ongoing assessment.

SOCIAL SERVICES 

assists the Patient and Caregiver(s) with any need for outside assistance or available sources (such as: Nursing Home Placement-when the time is right, or appyling for Medicare or Medicaid, etc)

Office Staff/Clerical Team 

Supports the Hospice Care Team, managing the day-to-day operations.

The Public Relations and Marketing Team

Work with different partners in the community bringing awareness to the different Hospice Diagnosis as well as the services of JCH.
